Abstract
The mucin-producing tumor of pancreas is characterized by an enlarged papilla with a dilated orifice and visible mucin oozing through orifice on endoscopy After 1982. Ohhashi et al. reported four unique cases of above characteristics, more than
50
cases
of mucin-producing pancreatic tumor have been reported in Japan. It has been recognized that not only carcinoma but also some benign lesions can produce these features; mucinous cystadenoma, mucinous cystadenocarcinoma, mucinous carcinoma, or a
cystic
lesion with a hyperplastic metaplastic mucous-secreting epithelium.
The number of patients with cystic tumor in pancreas is increasing because or recent development of image diagnostic techniques, but the classifications of these lesions are still controversial. Recently. a new entity, ductectatic type of
mucinous
cystadenoma was reported sporadically in Japan. this entity is characterized by the presence of a mass lesion in uncinate process of pancreas that represents cystic dilatation of a side branch of the main pancreatic duct, which contains thick
mucoid
secretions. atypia that are difficult to distinguish from mucinous cystadenoma or cystadenocarcmoma. But there have been no report of mucin-producing tumor and ductectatic type of cystadenoma of the pancreas in Korea.
We-reported a 76 year-old male diagnosed as mucin-producing ductectatic type of mucinous cystadenoma of pancreas. 14 months earlier, he had diagnosed as obstructive jaundice and sepsis due to the cystic tumor of the pancreas head confirmed by
percutaneous transhepatic cholangiography and abdominal CT scanning. We had proposed the needle aspiration cytology and operation, but he refused our proposal and discharged. 14 months later, he visited again our hospital for the purpose of
reevaluation
of the pancreatic tumor. Abdomen ultrasonography and CT scanning performed on his second admission revealed markedly dilated main pancreatic duct and a 7 cm lobulated cystic mass, and an enlarged papilla with expelled mucin from a patulous
orifice
was
noticed on duodenoscopy.. Endoscopic retrograde pancreatogram showed the dilated main pancreatic duct with irregular filling defects and cholangiogram performed through the PTBD tube revealed extrinsically compressed.distal common bile duct and
markedly

with the main pancreatic duct and microscopic findings revealed the cysts surrounded by the dense fibrous wall, and the wall of an internal cyst composed by rather atypical stratified columnar epitheliums which grows papillary pattern.
